My name is Marlene Derian Robertson. I am a graduate student in Organizational Leadership in the Graduate School of Education and Psychology at Pepperdine University in Los Angeles, California. I am conducting a survey on Six Sigma and Systems Thinking, an integrated approach to quality improvement for my dissertation. This study is being conducted in partial fulfillment of the requirements for the degree of Doctor of Education in Organizational Leadership under the supervision of June Schmieder-Ramirez, PhD.

Your participation in this research project is voluntary and will be asked to share your experience with quality management programs, specifically Six Sigma programs. The survey is open until July 15 and will be administered online using SurveyGizmoTM and should only take about 20 – 30 minutes; you may discontinue your participation at any time.

Your survey responses will be strictly confidential. All answers will be kept confidential. The results of this survey will be presented publicly at Pepperdine University. Only aggregate (group) results will be presented or documented, not individual answers.

By completing the survey, you will have an opportunity to obtain a copy of the summary survey report by clicking on the link at the end of the survey. You will also have an opportunity to enter into a drawing for a $50 (USD) gift card from Participation in the drawing is voluntary. Should you choose to participate, your contact information for the drawing will be collected and kept independently from your answers. Upon the final defense is completed, your email address will be permanently deleted from the researcher’s data collection.
